As families prepare for the 2026-2027 school year, Ada Schools reminds parents and guardians that Ohio law requires all students to be up to date on required immunizations to attend school. These vaccinations are not only a legal requirement but also play an important role in protecting the health and well-being of students, staff, and the broader school community.

The following immunizations are required for students entering specific grade levels:

Kindergarten

  • 5 doses of DTaP

  • 2 doses of MMR

  • 2 doses of Varicella

  • 3 doses of Hepatitis B

Seventh Grade

  • Tdap

  • Meningococcal (ACWY)

Twelfth Grade

  • Meningococcal (ACWY)

While vaccinations are required for school attendance, Ohio law allows exemptions for medical reasons as well as reasons of conscience, including religious convictions. Families requesting an exemption must provide appropriate documentation to the school, either through an exemption form or a medical statement from a physician.

Immunization records and exemption documentation may be submitted to the school office.

Vaccine Locations: To schedule your student for their required vaccinations, consider these options:

  • Allen County Health Department: online appointments can be made at www.allencountypublichealth.org.

  • Wyandot County Health Department: immunization appointments are offered on Monday and Friday. An appointment can be made by calling 419-294-3852.

  • Hancock County Health Department: immunizations are offered by appointment only. An appointment can be made by calling 419-424-7441.

  • Logan County Health Department: Immunizations are offered on Mondays and Tuesdays from 9 AM to 12 PM and 1 PM to 3:15 PM. Call (937) 651-6186 to schedule.

  • Kenton Hardin Health Department: immunization clinics are not currently being held.

  • ONU Healthwise Pharmacy: offer vaccinations for students age 5 and older
